Advancing Better Living for Elders.
CAPABLE is a free program that helps eligible older adults improve safety, mobility, and independence at home through support from a Registered Nurse, Occupational Therapist, and home modifications.
What is CAPABLE?
A 4-5 month program that helps older adults improve:
- Safety in the home
- Independence with daily activities
- Overall health and quality of life
Participants work with:
- Registered Nurse (RN)
- Occupational Therapist (OT)
- Handy Worker (home modifications)
There is no cost to participate. The program is fully funded through grant support and no insurance is required.

The CAPABLE program helps older adults stay safe, independent and comfortable in their own homes. By focusing on practical goals that matter most to you, CAPABLE supports daily activities, reduces fall risks and makes the home environment safer and easier to navigate. Participants often experience improved strength, confidence and quality of life while learning skills that support long-term health and independence. By preventing injuries and addressing challenges early, the program may also help reduce hospital visits and overall healthcare costs -- allowing you to focus on living well at home for as long as possible.
Who is eligible?
- Adults aged 60 years or older.
- Cognitively intact or experiencing only mild cognitive impairment, and willing to actively participate in goal setting and action planning.
- Experiencing difficulty with at least one activity of daily living (ADL) or two or more instrumental activities of daily living (IADLs).
- Eligible for Medicaid or has a household income at or below 150% of the Federal Poverty Level (FPL) based on the current calendar year guidelines.
- Not currently receiving other paid services that duplicate the services provided through CAPABLE.
