If you or a loved one would like to remain independent at home, but need help with care, Schofield’s award-winning Adult Day Health Care program can help. The Adult Day Health Care program enables someone to remain living in their community while they receive the health and rehabilitative services they need.
Adult Day Health Care often reduces emergency room visits and hospitalizations while also providing education, support and respite for family members who are caregivers. The program provides a supportive group environment with structured leisure-time activities that include cooking, board games and arts-and-crafts.
The Schofield Adult Day Health Care Program operates two sessions, five days a week, Monday through Friday, 8:00 a.m. to 1:00 p.m. and 1:30 to 6:30 p.m., and one session on Saturdays from 11:00 a.m. to 4:00 p.m. Most registrants attend a minimum of five hours.
Schofield’s Adult Day Health Care Program is the largest of its kind in Upstate New York. It is the only program in Erie County that has both a morning and afternoon session and is open 6 days per week.
Schofield’s Adult Day Health Care Program is proud to have been chosen as the Program of Excellence Award from the Network in Aging and the Innovation of the Year Award for Programming from the NYS Adult Day Health Care Council.
