Per-Diem TBI Service Coordinator
Locations: 2757 Elmwood Ave Kenmore, NY 14217
The TBI Service Coordinator (SC) provides case management to participants and prospective participants of the Home and Community Based Service/Traumatic Brain Injury Waiver Program of New York State (HCBS/TBI Waiver). TBI SC coordinates and monitors all of the services in the participant’s Service Plan, which may include Medicaid State Plan Services, as well as educational, vocational, social and medical services. The SC works with the participant to increase their independence, productivity and integration into the community while maintaining their health and welfare.
Qualifications include:
At least one year experience providing case management to individuals with disabilities.
Experience working with TBI Participants is a plus.
Must possess a valid driver’s license and have a car available for use.
